Before you begin

SS-5 is the Social Security Administration form used to apply for a Social Security card. This flow is optimized for a new card for a child.

What SSA usually wants for a child

  • Proof of age, usually a birth certificate if one exists.
  • Proof of U.S. citizenship for a U.S.-born child.
  • Proof of the child's identity, or if the child is very young, proof of the parent signing the application.
  • Sometimes proof of custody or responsibility for the child.

Original documents only

SSA says all evidence must be an original or a copy certified by the issuing agency.

  • Photocopies are not accepted.
  • Notarized copies are not accepted.
  • Plan on bringing at least two separate documents.

Helpful to have ready

  • Child’s legal name and date of birth
  • Place of birth (city + state)
  • Parents’ names (and SSNs if available)
  • Mailing address where you want the card sent
  • Your original supporting documents before you head to SSA or mail anything
