Before you begin
SS-5 is the Social Security Administration form used to apply for a Social Security card. This flow is optimized for a new card for a child.

What SSA usually wants for a child
- Proof of age, usually a birth certificate if one exists.
- Proof of U.S. citizenship for a U.S.-born child.
- Proof of the child's identity, or if the child is very young, proof of the parent signing the application.
- Sometimes proof of custody or responsibility for the child.
Original documents only
SSA says all evidence must be an original or a copy certified by the issuing agency.
- Photocopies are not accepted.
- Notarized copies are not accepted.
- Plan on bringing at least two separate documents.
