Names

Parent/guardian name

Avoid delays and mismatches

  • Use your name exactly as it appears on your Social Security card (legal name).
  • Avoid nicknames. Keep spaces, hyphens, and apostrophes the same (for example, “De La Cruz”, “Smith-Jones”, “O’Neil”).
  • Compound last names: enter the full last name in the Last name field.
  • Middle name is optional—use your full middle name or middle initial if that’s what you use on official records.
  • If your name changed recently (marriage/divorce), use the Social Security card name until it’s updated.

Child name

Match the child’s Social Security card

  • IRS i4547 (Part II—Name) says to check that both the name and SSN on Form 4547 match the child’s Social Security card.
  • Use the child’s legal name (including spaces, hyphens, apostrophes, and compound last names).
  • Middle name is optional—use the full middle name or initial if that’s what appears on official records.

Dates

Parent/guardian date of birth

Enter the parent/guardian date of birth. If you’re unsure about formatting or the date differs across documents, add your question/context below.

Format: YYYY-MM-DD.

Child date of birth

Enter the child’s date of birth exactly as it appears on official records. This is used for Form 4547 Part II and helps determine age-based eligibility (for example, the Pilot Program DOB window).

  • Format: YYYY-MM-DD.
  • IRS i4547 Pilot Program eligibility includes being born after Dec 31, 2024 and before Jan 1, 2029.
  • If documents disagree, add context in the question box.

Phone

Phone

Enter a daytime phone number where you can be reached. We use this to contact you if there’s a question about your submission.

  • US numbers: include area code (10 digits).
  • Non‑US numbers: include country code (E.164 format like +44… or +1…).
  • This is required by the app so we can contact you quickly if needed.

Enter an email address you check regularly. If we have a question about your submission, email can be the fastest way to reach you.

Addresses (US & foreign)

Street address

Enter your mailing street address (number and street). IRS instructions for Form 4547 say to enter a box number only if your post office doesn’t deliver mail to your street address.

If you receive mail at both a street address and a P.O. box, use the street address here and add any special delivery details in the question box.

Apt / suite

Add your apartment, suite, or unit number (if any). Leave blank if it doesn’t apply.

County

County is optional. IRS instructions note that the word “county” includes a parish or any other equivalent subdivision of a state or territory of the United States.

State

Foreign country

Foreign province/state/county

Foreign postal code

Postal code for your foreign address. IRS instructions say to follow the country’s practice for entering the postal code.

Same address as parent/guardian

If the child’s home address is the same as the parent/guardian (Part I), check this box. Otherwise, complete the child’s address fields.

Child street address

If the child doesn’t live at the parent/guardian address, enter the child’s street address. IRS i4547 also notes for addresses to enter a box number only if the post office doesn’t deliver mail to the street address.

Pilot program contribution

Pilot program contribution election

Only check if the child qualifies and you want the $1,000 Pilot Program Contribution. Requirements include DOB window, U.S. citizenship, qualifying child status, and a valid SSN (see instructions).

Trump account options

Contributing to a Trump account (interest only)

This question helps us understand what families want to do. This app doesn’t make contributions yet.

  • IRS instructions describe several contribution sources during the growth period (including the $1,000 pilot program contribution, employer contributions, and “other sources” like parents or other people).
  • The IRS instructions say contributions cannot be made before July 4, 2026.
  • The IRS instructions say the total of “other contributions” are generally subject to an annual limit of $5,000 (with cost-of-living adjustments after 2027).

Contribution amount (interest only)

If contributions become available, this amount is meant to reflect your intended annual contribution amount from “other sources” (for example, a parent/guardian).

IRS i4547 describes an aggregate annual limitation of $5,000 for “other contributions” during the growth period (with cost-of-living adjustments after 2027).

Using a brokerage/financial app (interest only)

This question helps us understand what integrations people want. This app doesn’t connect accounts yet.

  • IRS instructions say Trump accounts must have a trustee (a bank or an IRS‑approved nonbank trustee).
  • During the growth period, Trump accounts can only be invested in eligible investments (generally mutual funds or ETFs that track an index of primarily U.S. companies and meet certain requirements).
  • Which brokerages/apps support Trump accounts may depend on trustee/platform availability.
