- Avoid nicknames. Keep spaces, hyphens, and apostrophes the same (for example, “De La Cruz”, “Smith-Jones”, “O’Neil”).
- Compound last names: enter the full last name in the Last name field.
- Middle name is optional—use your full middle name or middle initial if that’s what you use on official records.
- If your name changed recently (marriage/divorce), use the Social Security card name until it’s updated.
